Register with us as a New Patient

When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form, these must be completed before you see the Doctor.

Practice Boundary

As a guide, our practice area serves Pollok and surrounding areas which includes G51, G52, G53, G41, G43, G46. We are unable to cover anywhere lying in the East Renfrewshire Council area of G46 as District Nursing services etc cannot be provided cross border of different council regions since each has its own Health and Social Care Partnership.

Acceptance on the list

Acceptance on to the list is as at the discretion of the Doctor. We may also ask you for proof of identity.(Please refer to our registration policy below and why we ask for this).

There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case simply choose another GP in your local area.

Other Information

If you prefer a particular type of doctor, e.g. female for a female patient and vice versa we will note this and do our best to respect your choice. However, not all the doctors in the practice provide all services and specific doctors may not be immediately available. 

It is important that we meet you as soon as possible to discuss your medical history and medication, and assess your health needs. Please remember it may take several weeks for your case notes to be transferred from your previous doctor.
